I grant permission to CJW Services, Inc (dba KidStage) to use and publish any photos/recordings that may include my child/dependent. These images/recordings may be done in any medium now existing or subsequently developed, on the KidStage website and/or any social media platforms.

I waive my right to inspect or approve any images, recordings, editorial text or copy that is used and release and discharge CJW Services, Inc (dba KidStage) and all of its agents/contractors/employees from any and all claims arising out of use of the images/recordings, including any claims for libel, invasion of privacy, and the like. I waive any rights, claims or interest and I understand that there will be no financial compensation from CJW Services, Inc (dba KidStage) for the use of these images/recordings.

I agree that if my child/dependent damages/breaks/takes any property that does not belong to my child/dependent/family that I will be financially responsible for these items.

I certify that there are no health-related reasons or problems which preclude my child/dependent’s participation in KidStage.  I certify that my child whom I am registering for the program does not have any contagious diseases to my knowledge and I assert that if he/she should become ill that I will not allow my child to attend KidStage class until he/she has been cleared by a licensed physician.

Should my child be injured while participating in KidStage, I hereby authorize CJW Services, Inc. (dba KidStage) and its staff or representatives, the power of care of my child, including, but not limited to, the power to authorize all medical, dental and hospital care for my child, and the power to execute all documents and releases to obtain such care should I be unavailable at the time of injury. This delegation and grant of power of care for my child shall be effective from the date and time of the first class until 2 hours after the final performance/class.

In voluntarily registering my child/dependent in KidStage, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assign as follows:
(A)I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my child/dependent’s death, disability, personal injury, property damage, property theft, or actions of any kind which may occur to my child/dependent including travel to and from KidStage THE FOLLOWING ENTITIES OR PERSONS (Releasees): CJW Services, Inc. (dba KidStage), KidStage Officers, KidStage Directors, KidStage Instructors, KidStage Representatives or Volunteers for KidStage.
(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in (A) from any and all liabilities or claims made as a result of participation in KidStage, for any cause. I acknowledge that KidStage may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by facilities, forces of nature, participants, instruments, equipment, illnesses, props, furniture, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, instructors, directors, spectators, and audience members. The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

KidStage Behavior Expectations

We encourage our participants to be loud, energetic, and have fun!  We will be playing a lot of theater games and learning about theater terminology and characterization.  We will also spend a large part of the class time rehearsing for our final production.  Therefore, participants are required to behave in a classroom-conducive manner.  This includes, but is not limited to, listening and following the Instructor’s requests and instructions, keeping one’s hands to themselves and not touching others in class nor any property in class that does not belong to them, refraining from the use of inappropriate language, refraining from teasing and negative comments to others in class, never leaving the class without permission from the instructor, and not disrupting the rehearsal process and class.

If your child requires/has an aide with them during the school day, you must have an aide accompany them to KidStage.

This is especially important during the rehearsal times.  In theater there are times when the instructor will be working on a scene with a group of actors, and not all actors will be on stage at that time.  It is important that those that are not in the scene are respectful, quiet, and not disrupting the rehearsal.

When a student violates or disrupts the class, they will be given a warning.  If the disruptive behavior ceases, then no other action is taken.

If they continue to act in an inappropriate way, in a manner that disrupts the learning in the class, or may physically endanger others in the class or property located in the class, they will be placed in a timeout situation and their parents will be contacted after class to notify them of the behavior and to collaborate with the parent on solving the behavior problem so it does not continue.

Parents must be responsive to the instructor and take the situation seriously.  We typically ask for them to talk to their children at home about the behavior and use any leverage they may have to motivate the child to change their behavior for the next class.

If the problem persists in the next class after the initial instance of disruptive behavior exhibited by the child, the child will be placed in time out again and at the end of class the parents will be contacted again.  At this point there are 2 final solutions/paths to take.  1) The child is expelled from KidStage and no longer continues in the program, or 2) The parents agree to have one adult representative attend the remainder of the classes from start to finish to control the behavior of their child.  If the parents do not agree to #2, then #1 is the path that is taken and in these cases NO REFUNDS are provided.

In the case of expulsion from the class, no refunds will be given.  Expelling a cast member creates a lot of work for our instructor as well as wreaks havoc with the rest of the class since the casting will have to be adjusted to accommodate for the loss of an actor.  Also, if parents are not responsive to our communication, and show no interest in collaborating with our Instructor on solving the behavior issues their child is exhibiting, the child will be expelled from class without a refund.

If classes are cancelled due to but not limited to the following things: school closings, force majeure, pandemics, weather, etc, I understand that no refunds nor partial refunds will be given.  There are limited spots available for our courses and we secure your child’s spot upon receiving your registration.  If for ANY reason you choose or your child cannot otherwise participate in the program for which you’ve registered, you will not receive a refund. We do not assume any liability for the health of your child, if your registered child is injured and cannot participate, you will not receive a refund.

You agree and approve that you will receive no refund for any missed classes (e.g. courses that are partially completed) that must be canceled because of school closures, pandemics, or any other major events that require us to cancel our class outside of our control.

Timeliness of Pick-Up

You agree that a designated adult will pick up your child from our class on time at the end of class.  If you are late for pick-up, you could be assessed a fine of $1/minute that you are late past the 5 minute grace period.  This fee will be paid directly to the Instructor for the class if it is incurred.  Our instructors have other jobs and commitments and if they are late it can adversely impact them financially or professionally.  In the event you do not pay any late fees you are assessed we reserve the right to expel your child from the rest of the classes and final production.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I KNOWINGLY AND FREELY ASSUME ALL THE RISKS, BOTH KNOWN AND UNKNOWN EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS, AND ASSUME FULL RESPONSIBILITY FOR MY CHILD/DEPENDENT’S PARTICIPATION. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND I WILL HAVE NO RIGHT TO MAKE A CLAIM OR FILE A LAWSUIT AGAINST THE RELEASEES UNDER ANY CIRCUMSTANCE. I SIGN ON MY OWN FREE WILL BY CLICKING THE BOX ON THE KIDSTAGE WEBSITE AND REGISTERING MY CHILD FOR KIDSTAGE.
